フロー図に、調査の長期化と再調査の遅れを課題⑨として追加した。「調査の実施」の枠に課題⑨のマーカーを付け、課題・論点の一覧に項目を足した（リンク先＝令和8年6月定例会（4）「いじめ重大事態の対応遅れと再調査について」）。あわせて、フロー図ページの「課題①〜⑧」の記述を「課題①〜⑨」に更新した。
